Claims

General Claims Procedure:

  1. Report the event to the police if someone has been hurt, in the case of an accident or in the event of any criminal acts such as theft.
  2. DO NOT admit fault, liability, responsibility or make any offers or payments.
  3. Download a claim form here or request one from us
  4. Complete the form in full and either email it to admin@medigro.co.za or Fax it to 011 483 2533
  5. Very Important: You must send us your claim form within 30 days of the incident.
Motor Accident
  • Fully completed claim form
  • Clear copy of the valid card type driver license of driver at the time of the incident or a copy of the driver license of the person who last used the vehicle
  • Copy of the Drivers ID
  • Copy of vehicle registration papers (If not yet received and noted on schedule)
  • No need to submit or obtain quotations. The appointed Assessor will do the necessary Audatex Quotations
House Contents Section
  • Fully completed claim form
  • Quotations for replacement/ repair of claimed items
  • Proof of ownership / valuations of stolen items – Jewellery items ect.
  • Damage report from service provider stating cause of damage – On repairable items (Lightning/Power-surge or accidental damage)
All Risk Items
  • Fully completed claim form
  • Quotations for replacement/ repair of claimed items
  • Proof of ownership / valuations of stolen items – Jewellery items ect.
  • Damage report from service provider stating cause of damage – On repairable items (Lightning/Power-surge or accidental damage)
  • Written confirmation of black listing from service provider with IMEI number and ITC number – Cell phones and tablets (expect if Wi-Fi only)
